Daily Attestation

Dear Parents and Guardians,
You are required to complete a daily COVID-19 witness form (Attestation Form) for each day your student is scheduled to attend school in person. This form will be on your Genesis account only on the days your children are scheduled to be in school. The form includes questions about your recent travels and any COVID-19 symptoms you, your child or children may be experiencing. If the form is not completed, your student will not be able to enter the school.

En Español: Es necesario que usted complete el formulario diario de testimonio de COVID-19 por cada día que su estudiante va a asistir a la escuela enpersona. Este formulario se encuentra en su cuenta de GENESIS, y solo aparecera en los dias que su estudiante este programado para ir a la escuela en persona. El formulario incluye preguntas acerca de sus viajesrecientes y de cualquier síntoma de COVID-19 que pueda estar experimentando usted, el estudiante o su familia. Si el formulario no secompleta su estudiante no podrá ingresar a la escuela.
