Greetings Plainfield Families,


The Spring athletic season is approaching quickly. During the spring season we offer baseball, softball, boys tennis, coed track and field and boys volleyball for students in grades 6-12.  To try-out for a school athletics team the student MUST have a completed sports physical packet.


HS Spring Sports Try-outs start on Monday, March 16th  

Try-outs/practices/games will occur at the following locations this spring season:

MS Baseball – Cedarbrook School

MS Softball – Maxson MS

HS Tennis – Donald Van Blake Tennis Courts

MS/HS Track & Field – Hub Stine Field

HS Boys Volleyball – PHS


Sports Physical Information

To participate in athletics during the 2022-23 school year you must do the following:


1.       Download or obtain a Sport Physical Packet which contains the Athletic Consent form and the Physical Exam forms.  Parent/guardian must complete the consent form and the health history portion of the physical forms.  These are the only forms that can be used.

2.       Get a physical exam done by your Primary Care Provider (MD, DO, APN, or PA). Please bring the completed health history form to the exam. The provider must complete and document all parts of the exam.  Physicals are valid for 365 days.  All athletes must have a current valid physical to try out for sports.

3.       Submit the Physical Packet to your child’s school nurse or bring directly to the HS school nurse during school hours.


*If you are planning on playing a spring sport, your completed sports physical must be turned in by Wednesday, February 20th to be cleared by the start of try-outs.  As a reminder once try-outs have started no more students will be accepted to that team.    


  1. Sports physicals are good for 365 days, if your student played a fall or winter sport and the sports physical is still valid you need to complete the Health History Update forms and turn into the school nurse by Wednesday, February 15th.  Any questions regarding your child’s sports physical date please reach out to your child’s school nurse.

To Try out for a PHS Athletic Team

You MUST have a cleared sports physical turned into your school nurse.
You MUST take the ImPact concussion baseline test.
All completed sports physicals and/or health history updates must be handed into the school nurse in order to be cleared.

Sports Physical Forms are also available in the Nurse's Office, Athletic Training Room, and Athletics Office.
Any questions regarding sports physicals and health history update forms should be directed to the school nurse.
During July & August, sports physical packets can be dropped off at PHS.
